2018-19 Student-Athlete Paperwork

It is never too early to plan for your potential participation in one or more of Midview’s 2018-19 athletic programs. Below is list of resources that a Middie student-athlete and his/her family needs to be aware of before the beginning of their upcoming sport season.

Coaching Directory: Contact your potential coach with any questions you may have prior to the season. - Coaching Directory

Pay-to-Participate: Midview Local Schools Pay-to-Participate, revised in 2015-16, will remain in effect for 2018-19. Payments can be made directly to the Athletic Office or Board of Education throughout the summer. In addition, online payments can be made at https://payforit.net/. Fees must be paid in their entirety prior to any student-athlete participating in a regular season event. Unfortunately, financial support is not available with the athletics office. Refunds are available only in limited circumstances. - PTP Brochure (PDF)

Active Physical: Every student-athlete must have an active physical on file in the Athletic Office. This six-page document must be completely filled out and returned prior to anyone participating in a practice. Please keep a copy. It is not the Athletics Office's job to make copies for you at a later date. Physicals are good for one calendar year. - 2018-19 Physical Form (PDF)

Other Athletic Forms: Beginning in 2018-19, Midview Athletics has shifted its Concussion, Code of Conduct, Insurance Waiver, Emergency Medical and Sudden Cardiac Arrest forms online. These forms will connect directly to PowerSchool through software called OneView. The OneView portal is expected to be available on or before July 23. No paper forms will be accepted this year except the OHSAA Physical. You can create you account here: https://midview.esvportal.com/
