MIDVIEW PHYSICAL AND PAY-TO-PARTICIPATE INFORMATION SHEET

FOR THE 2008-09 SCHOOL YEAR

 

PAY-TO-PARTICIPATE FEES (PER ATHLETE) –

MIDVIEW HIGH SCHOOL

1st Sport - $150

2nd Sport - $100

3rd Sport & beyond – No Charge

 

MIDVIEW MIDDLE SCHOOL 

1st Sport - $100

2nd Sport - $75

3rd Sport & beyond – No Charge

 

There is a family cap for MHS, MMS, or a combination of the two schools at $600.

 

 

MIDVIEW HIGH SCHOOL AND MIDVIEW MIDDLE SCHOOL

ATHLETIC PHYSICALS

 

Athletic Physicals will be offered by Dr. Barker and his Tri-City Medicine doctors on Thursday, June 5 in the Midview High School APR.  The cost is $20, and all checks should be made out to the Midview Athletic Department.  All of the doctors and nurses are donating their time for the athletes of the Midview Schools.  Each child should bring a completed and signed physical form from their parents.  The physical information on page 2 will be completed by the doctors.  At the end of the physical, the athlete will turn in the completed physical form at the check-in table, and copies will be available to students who request them.  The schedule for the physicals is as follows:

 

6 – 7 pm – Juniors and Seniors

7 – 8 pm – Freshmen and Sophomores

8 – 9 pm – 7th and 8th Graders

 

These physicals are for Midview students only, as the doctors are donating their services to the students of the Midview School District.
 

EIGHTH GRADE FOOTBALL

When: Thursday, July 31, 2008

Meeting: Middle School Gym

For: Parents and Players

Time: 5:30 p.m.—6:30 p.m.

Practice: Begins August 4, 2008

Time: 5:00 p.m.—7:00 p.m.

Where: Practice Field behind Midview Middle School

Bring: Shorts, T-Shirt, Athletic Shoes, Water Bottle

Forms: Physical, Waiver, and Emergency Medical

(These forms must be filled out, signed, and turned in at the meeting on July 31, 2008. You will not be able to practice until these forms are in.)

 

Fee to participate: $100

This fee is non-refundable.

Fee will be payable before the player receives his equipment.

 

Checks may be made payable to:

SEVENTH GRADE FOOTBALL

When: Friday, August 1, 2008

Meeting: Middle School Gym

For: Parents and Players

Time: 5:30 p.m.—6:30 p.m.

Time: 3:30 p.m.—5:30 p.m.

Where: Practice Field behind Midview Middle School

Bring: Shorts, T-Shirt, Athletic Shoes, Water Bottle

Forms: Physical, Waiver, and Emergency Medical

(These forms must be filled out, signed, and turned in at the meeting on August 1, 2008. You will not be able to practice until these forms are in.)

Where: Midview High School in Kubuski Gymnasium

Bring: Shorts, T-Shirt, Tennis Shoes, Water Bottle

Forms: Physical, Waiver, and Emergency Medical

(These forms must be filled out, signed, and turned in on the first day of tryouts. You will not be able to tryout until these forms are in.)

 

Fee to participate: $100

This fee is non-refundable.

Fee will be payable before the player receives his equipment.

Tryouts 2008

Where: Midview High School in Kubusky Gymnasium

Bring: Shorts, T-Shirt, Tennis Shoes, Water Bottle

Forms: Physical, Waiver, and Emergency Medical

(These forms must be filled out, signed, and turned in on the first day of tryouts. You will not be able to tryout until these forms are in.)

Where: Behind Midview Middle School

Bring: Shorts, T-Shirt, Running Shoes, Water Bottle

Forms: Physical, Waiver, Emergency Medical

(These forms must be filled out, signed, and turned in on the first day of practice. You will not be able to practice until these forms are in.)
