Meet Final Forms
Midview Local School District partners with FinalForms, an online forms and data management service. FinalForms allows parents/guardians to complete and sign Emergency Medical forms, Demographic forms, Athletic forms, and more for students. FinalForms also saves data from year to year and season to season, meaning there is no need to enter the same information twice! FinalForms also pre-populates information wherever possible, for all students in one family, saving precious time.
Data may be reviewed at any time to verify it is current. All forms are required to be updated once per year and after any change.
New Student Registration (Grades 1-12) & Returning Student Information
New in-district students in GRADES 1-12 can be registered for the upcoming school year beginning May 1. Please follow all steps as outlined below to register an in-district student in the Midview Local School District or update any information that may have changed for the present or upcoming school year. Parents must update all required forms annually in order for students to attend school.

Little Middies Preschool Peer Model Registration
Servicing children with special needs (ages 3-5) alongside typically developing Peer Models (age 4), Little Middies Preschool follows a developmentally appropriate and multi-sensory curriculum aligned with Ohio’s Early Learning Content Standards.
Families interested in their child attending Little Middies Preschool as a peer model for the 2023-2024 school year MUST FIRST complete the Preschool Interest Form. Students eligible to be a Preschool Peer Model must have their 4th birthday by August 1st of the school year they are registered for AND live within the district. Please follow all steps as outlined below to register a preschool peer model student in the Midview Local School District.

Kindergarten registration will open on March 13, 2024, followed by Kindergarten Welcome Night at North Elementary on March 14 from 5:30-6:30 p.m. Students eligible for Kindergarten must have their 5th birthday by August 1st of the school year they are registered for. Please follow all steps as outlined below to register an in-district Kindergarten student in the Midview Local School District. If your Kindergarten student is open-enrolled, please see the Open Enrollment information below first.

Open Enrollment Information
Returning Open Enrollment Applications
In order to be considered for Open Enrollment, an application must be filled out for each individual student each school year and submitted by the student’s legal guardian during the application window. The window to submit a Returning Open Enrollment Application will open on February 21, 2024, and remain active until March 22, 2024. Current open-enrolled families will receive notification through their FinalForms email with the next steps.
New Open Enrollment Applications
The district will begin accepting New Open Enrollment Applications on May 1, 2024, through May 24, 2024. The New Open Enrollment application for the 2023-2024 school year is digital and can be found below.
Filling out an application does not guarantee that a student will be accepted into the district. Space is limited and students will be considered based on current staffing and the time in which an application was filled out and submitted by the legal guardian.
