Midview Local School District partners with FinalForms, an online forms and data management service. FinalForms allows parents/guardians to complete and sign Emergency Medical forms, Demographic forms, Athletic forms, and more for students. FinalForms also saves data from year to year and season to season, meaning there is no need to enter the same information twice! FinalForms also pre-populates information wherever possible, for all students in one family, saving precious time.

Data may be reviewed at any time to verify it is current. All forms are required to be updated once per year and after any change.

New in-district students in GRADES 1-12 can be registered for the upcoming school year beginning July 1. Please follow all steps as outlined below to register an in-district student in the Midview Local School District or update any information that may have changed for the present or upcoming school year. Parents must update all required forms annually in order for students to attend school.

Servicing children with special needs (ages 3-5) alongside typically developing Peer Models (age 4), Little Middies Preschool follows a developmentally appropriate and multi-sensory curriculum aligned with Ohio’s Early Learning Content Standards.

Families interested in their child attending Little Middies Preschool as a peer model for the 2024-2025 school year MUST FIRST complete the Preschool Interest Form. (NOTE: Families will be notified of acceptance, denial, or waitlist status by the Little Middies Preschool Administrative Secretary after April 9.)

Students eligible to be a Preschool Peer Model must have their 4th birthday by August 1st of the school year they are registered for AND live within the district. Please follow all steps as outlined below to register a preschool peer model student in the Midview Local School District.

Kindergarten registration will open on March 13, 2024, followed by Kindergarten Welcome Night at North Elementary on March 14 from 5:30-6:30 p.m. Students eligible for Kindergarten must have their 5th birthday by August 1st of the school year they are registered for. Please follow all steps as outlined below to register an in-district Kindergarten student in the Midview Local School District. If your Kindergarten student is open-enrolled, please see the Open Enrollment information below first.

To be considered for Open Enrollment, an application must be filled out for each student each school year and submitted by the student’s legal guardian during the application window. 

The district will begin accepting Open Enrollment Applications on May 1, 2024, through May 31, 2024. The Open Enrollment application for the 2024-2025 school year is digital and can be found below.

Filling out an application does not guarantee that a student will be accepted into the district. 

Class sizes and staffing levels are determined based on the number of Midview residents the district must serve. Once these numbers are established, open-enrolled students are accepted as needed to meet grade-level capacity limits. 

Preference is given to returning open-enrolled students first.

PLEASE NOTE: After the initial open enrollment application period ends, students will be placed on a waitlist if not accepted. Applications are then reviewed periodically throughout the summer as movement occurs within the district (in-district students are withdrawn/families move into the district). Due to increased interest and the limited amount of space, families considering open enrollment into Midview Local Schools are also required to enroll in their home district.

Open enrollment applications WILL NOT be accepted after the May 31, 2024 deadline.
