FinalForms registration for the 2022-23 school year is now open to Midview Local School District families who will have student-athletes entering grades 7-12. [FinalForms Site]
To register for athletics for next school year, simply login into FinalForms and click the big blue registration button at the top of your login page and follow all directions.
All forms must be completed and an up-to-date physical must be on file (either digitally uploaded into FinalForms or a hard copy in the athletic office) in addition to being on a FinalForms roster for that sport to participate in any official athletic activity.
For those that are currently training during the off-season, we encourage you to complete this process as soon as you can so that our coaches have the most accurate rosters and emergency contacts heading into Summer.
